To have a prescription card sent to you, the e-mail you send us must contain your Name and Address (with zip code) and the names of family members to be included in your family card program.

E-mail us at  seba99@healthmartamerica.com  and allow 14 days to get your personalized cards back in the mail. 

You may use our Jet Issue card as your temporary drug card until you receive your personalized cards.  

Non-Profit and For-Profit Marketing Affiliates Applicants, email us with your initial request providing all of your contact information.
