FULL PAYMENT TO THE PHARMACY FOR ALL PRESCRIPTIONS NOT SUBSIDIZED BY PHARMACEUTICAL MANUFACTURERS OR NOT INCLUDED IN THE PROGRAM IS YOUR RESPONSIBILITY.

Unconditional 60 Day Guarantee: If for any reason you are not satisfied with the performance of the SEBA / HealthMart America Card or DestinationRx® card within 60 days of enrollment, simply return the card to us by mail for a full refund of any fees paid to issue your card. .
